ESCORT
Accompanying the individual on trips for medical appointments. It does not include the transportation of the consumer by the attendant.
HOME MANAGEMENT
Assistance with housekeeping activities that supports the individual’s health and safety.

- Washing dishes
PERSONAL CARE
Assistance with activities related to the physical health care of the individual:
- Bathing
- Dressing
- Grooming
- Routine hair & skin care
- Preparing meals

- Medication reminders/Assist
- Toileting
These are the programs available at Fidelity Home Health Services:
PRIMARY HOME CARE (PHC)
PHC is a non-technical, medically related personal care service available to eligible adults whose health problems cause them to be functionally limited in performing activities of daily living. Must be 21 or older or receiving PHC services through the Community Attendant Services program (CAS) eligibility. PHC services are provided by an attendant.
COMMUNITY ATTENDANT SERVICES (CAS)
CAS is a non-technical, medically related personal care service available to eligible adults whose health problems cause them to be functionally limited in performing activities of daily living. Adults and children with no age limit are eligible for CAS services. CAS services are provided by an attendant.
FAMILY CARE SERVICES (FC)
Non-technical attendant care service available to eligible adults who are functionally limited in performing activities of daily living. Must be 18 years of age and older. FC is a non-skilled. FC services are provided by an attendant and do not require the supervision of a nurse.
MEDICALLY DEPENDENT CHILDREN’S PROGRAM (MDCP)
MDCP provides services to support families caring for children and young adults who are medically dependent and to encourage de-institutionalization of children in nursing facilities.
PERSONAL CARE SERVICES (PCS)
Personal Care Services is a Medicaid benefit that provides assistance with activities of daily living (ADLs), instrumental activities of daily living (IADLs), and health-related functions to help clients with physical, cognitive, or behavioral functional limitations related to a disability, physical or mental illness, or chronic condition.
